Necessary documents for submission of an application in the Czech Republic and the Slovak Republic:

Patents, utility designs, industrial designs

• power of attorney (without a notarized signature - the document can be submitted at a later time); it is also possible to provide a general power-of-attorney
• declaration of the author(s) - information that the author(s) is/are or was/were an employee(s) of the applicant at the time of submission of the corresponding priority application - if this is not the case, it is necessary to submit the declaration of the author(s) that the author(s) has/have assigned the right to submit the given application to the applicant (without a notarized signature - the document can be submitted at a later time)
• priority document(s) (if priority is required) and transfer of priority rights (in a case where the name of the applicant stated on the priority document is identical with the name of the applicant stated on the Czech or Slovak application)

They need not necessarily be submitted at the same time as the application; they could be required only in course of further proceedings, nevertheless, it is better to submit the priority document(s) as well as the priority rights transfer document already at the time of submission of application

In the case of the national phase of international patent applications, priority documents and the documents of transfer of priority rights are not required by the Patent Office

We would like to inform you that in the Czech Republic, as well as in Slovakia, the date of entry into the national phase in the case of the international patent applications shall lapse 31 months from the latest priority period according to articles I and II.

• power of attorney - the same as applies to patents
• list of products and services (we classify according to the Nice International Classification)
• a priority document must be furnished within 3 months after data of notification
